Complaints

If You have comments, concerns or complaints about the Website, please direct them to Our Contact Us page.

Concerns about the actions or comments of Staff, should first be directed to the staff person who You are working with in case the issue can be resolved informally. As part of this process, the Director of Client Services may also try to resolve the concern You have informally. If informal resolution isn’t possible, then You should request a complaint form from the staff person You have been working with. This form must be completed in full to be accepted; incomplete forms will not be considered. Completed complaints forms are to be submitted according to the instructions on the form. You may include additional pages and attachments if necessary. If You are unable to fill out the complaints form for technological, logistical, or other reasons, or You require accommodation, please contact the Director of Client Services to make arrangements.

The services Director of Client Services will investigate complaints and respond in writing by email within 90 days of receiving the complaints form. If You are currently receiving services from CCHR, CCHR may suspend work related to Your file while the complaint is investigated.
